    Great link Attilio, one of the classic naval blunders caused the collision of the Victoria demonstrating the need for unequivocal and clear orders at all times. Probably as bad as the recent cruise ship running aground!

    The orders were clear and unequivocal, they were just wrong, many people realised they were wrong, but no-one questioned it (apart from Markham initially, and he followed it once the CinC gave him a bollocking by signal). A failure of an inflexible command system - I've been on the bridge of a (non-RN) ship where something similar almost happened, but the bridge crew were quite happy to point out the error to the skipper before things got too sticky.

    David you are correct the confusion as much as it was, came from Markham not understanding or rather not believing the order from Tyson. As usual writing in a hurry leads me to ill conceived sentences! Not unlike the admirals decision to turn the ships towards each other with inadequate room.
